CDC reservations for dog imports arriving via Philadelphia International Airport ONLY can be made here. Please allow two (2) business days for request and documentation review. A reservation is not complete until you receive a written reservation confirmation from our facility.

Certification of Foreign Rabies Vaccination and Microchip (CFRVM)(Required)

This form can be downloaded from the CDC website. Please upload your dog(s) Certification of Foreign Rabies Vaccination and Microchip (CFRVM) here. This specific form is required to enter the United States. Other certificates and forms, including pet passports and origin country rabies certificates, are not accepted. The certification(s) must be in English. Each dog listed for importation must have an individual CFRVM uploaded here.

Rabies Titer Results

Please upload your dog's Rabies vaccine titer lab work results here, if available. Each dog listed for importation must have an individual titer results uploaded here. If no results are uploaded, we will assume you do not have Rabies vaccine titer for your dog(s).

Additional Veterinary & Vaccine Records

All boarders at Gateway Animal Care Center must be up-to-date on Rabies, Distemper, and Bordetella vaccines, have negative fecal parasite testing, as well as endo- and ecto-parasite prevention. Please provide all of your pet(s) vaccination information and recent veterinary records HERE for our review. We will provide you with an estimate for any additional veterinary care required upon arrival for boarding at our facility.

Pet Disposition

Please advise if any of these dogs have known aggression with food, men, women, other dogs, etc. This will not affect your ability to make a reservation, only aid us in appropriately handling your dog(s) and minimizing their stress level.
